Visit diagnoses
The Visit Diagnoses card, in the Clinical section of the visit page, lists the diagnoses made during that visit. Each one shows the day it was diagnosed, the condition and its status. Expand opens the full table, which adds the ICD-10 code, the date it was resolved and the comment.
Visit diagnoses are for conditions found at a visit. Long-term conditions belong in the patient’s chronic diagnoses instead.
Adding a diagnosis
Section titled “Adding a diagnosis”Choose Add Diagnosis:
- Set the date of the diagnosis.
- Search for the disease by name. Type at least two letters: two or three find names in your language with a word that starts with them, four or more find them anywhere in a name, in any language. The best matches come first. Results show the ICD-10 code and name, and only conditions that are not chronic are offered.
- Optionally add a comment. The patient can read it.
The diagnosis is recorded under your name and your organization, and linked to the visit. You can add diagnoses only while the visit is open.
Resolving a diagnosis
Section titled “Resolving a diagnosis”A new diagnosis is Active. When the condition has cleared up, choose Resolve on its row. Its status changes to Resolved, with today’s date, and the condition is struck through. Resolving can’t be undone.
Anyone in your organization who can manage diagnoses can resolve one, not only the clinician who recorded it.
Deleting a diagnosis
Section titled “Deleting a diagnosis”If a diagnosis was entered by mistake, choose the bin icon on its row and confirm. The diagnosis disappears from the visit, and the patient no longer sees it. Anyone in your organization who can manage diagnoses can delete one. You can delete diagnoses only while the visit is open.
To record that a condition has cleared up, resolve it instead of deleting it.
Who sees what
Section titled “Who sees what”Viewing and managing diagnoses each need the matching permission. The card appears only to people who can view diagnoses; Add Diagnosis, Resolve and the bin icon also need permission to manage them. Someone who can manage diagnoses but not view them doesn’t see the card.
- Staff see the diagnoses made by their own organization, and only while the patient shares their clinical information with it. If the patient stops sharing, the card is empty and new diagnoses can’t be added.
- Patients can’t add, resolve or delete visit diagnoses. The patient app doesn’t show them yet. Through the API, a patient can list every diagnosis made for them, from every organization, including the clinician’s comment.
